About This Survey:

• The survey takes an estimated 6 minutes to complete
• The survey is open from July 6 – July 20 at 11:59 p.m. PST
• Survey responses are completely anonymous and will be analyzed in aggregate form by an impartial third-party consultant.
• You can provide optional contact information for a follow-up focus group or prize draw. Voluntary self-identification for these won’t impact the anonymity or validity of your survey responses.
